PATIENT PAPERWORK
Before your first visit, please download, print, and complete the New Patient Paperwork. Bring it with you to your scheduled appointment along with your insurance card and photo ID.
Patients who have been treated at Huntsville Pool and Land Therapy in the past need to complete the Returning Patient Paperwork if it has been less than a year since the last evaluation. For returning patients who have not been through an evaluation in over a year, please download and fill out the New Patient Paperwork.
